Byli jsme na toto t\u00e9ma \u010dasto dotazov\u00e1ni a v\u011bdecky jsme toto t\u00e9ma zkoumali.<\/p>\n\n\n\n<!--more-->\n\n\n\n<h2 class=\"wp-block-heading\" id=\"ersteinmal-m\u00fcssen-wir-\u00fcber-z\u00f6liakie-sprechen\">Nejprve si mus\u00edme promluvit o celiakii<\/h2>\n\n\n\n<p>Celiakie (ICD10: K90.0) je onemocn\u011bn\u00ed gastrointestin\u00e1ln\u00edho traktu zp\u016fsoben\u00e9 nesn\u00e1\u0161enlivost\u00ed lepku, kter\u00e9 m\u00e1 znaky alergie i autoimunitn\u00edho onemocn\u011bn\u00ed. Vyzna\u010duje se zejm\u00e9na chronick\u00fdm z\u00e1n\u011btem sliznice tenk\u00e9ho st\u0159eva v d\u016fsledku p\u0159ecitliv\u011blosti na slo\u017eky lepku, b\u00edlkoviny glutenu, kter\u00e1 se nach\u00e1z\u00ed p\u0159edev\u0161\u00edm v zrnech (semenech) mnoha druh\u016f obilovin (Wikipedia).<\/p>\n\n\n\n<p>\u010cetnost v\u00fdskytu celiakie se zna\u010dn\u011b li\u0161\u00ed: V\u011bt\u0161ina studi\u00ed p\u0159edpokl\u00e1d\u00e1 prevalenci&nbsp;<strong>Frekvence pod 1%<\/strong><sup><a href=\"https:\/\/euroconsum.eu\/cs\/publication\/glutenintoleranz_getue\/#fn:1\">1<\/a><\/sup>&nbsp;(1 ze 100 osob) obyvatel USA a Evropy. N\u011bmeck\u00e9 publikace uv\u00e1d\u011bj\u00ed je\u0161t\u011b ni\u017e\u0161\u00ed \u00fadaje (1 z 270 osob).<sup><a href=\"https:\/\/euroconsum.eu\/cs\/publication\/glutenintoleranz_getue\/#fn:2\">2<\/a><\/sup>. Zd\u00e1 se, \u017ee celiakie je \u010dast\u011bj\u0161\u00ed u lid\u00ed z\u00e1padokavkazsk\u00e9ho p\u016fvodu. U lid\u00ed africk\u00e9ho nebo \u010d\u00ednsk\u00e9ho p\u016fvodu je celiakie diagnostikov\u00e1na mnohem m\u00e9n\u011b \u010dasto.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"zu-h\u00e4ufig-um-wirklich-unwichtig-zu-sein\">P\u0159\u00edli\u0161 \u010dast\u00e9 na to, aby byly skute\u010dn\u011b ned\u016fle\u017eit\u00e9.<\/h3>\n\n\n\n<p>Pro&nbsp;<strong>Celiakie jsou jasn\u00e9 postupy digan\u00f3zy<\/strong>&nbsp;a p\u0159esto\u017ee se z\u00e1va\u017enost onemocn\u011bn\u00ed m\u016f\u017ee zna\u010dn\u011b li\u0161it, je \u010dasto spojeno se z\u00e1va\u017en\u00fdmi zdravotn\u00edmi pot\u00ed\u017eemi posti\u017een\u00fdch osob, v\u010detn\u011b.&nbsp;<strong>t\u011b\u017ek\u00fd pr\u016fjem<\/strong>&nbsp;a&nbsp;<strong>\u00dabytek hmotnosti<\/strong>.<\/p>\n\n\n\n<p>Krom\u011b toho existuj\u00ed n\u00e1znaky&nbsp;<strong>Vztah k dermatitid\u011b<\/strong>,&nbsp;<strong>Trpasli\u010d\u00ed vzr\u016fst<\/strong>&nbsp;a&nbsp;<strong>Pubertas tarda<\/strong>&nbsp;(MKN-10: E30.0),&nbsp;<strong>T\u011bhotensk\u00e9 komplikace<\/strong>&nbsp;a&nbsp;<strong>Potraty<\/strong>, jako\u017e i&nbsp;<strong>Asplenia<\/strong>&nbsp;(MKN-10: D73.0).<\/p>\n\n\n\n<p>Prevalence ni\u017e\u0161\u00ed ne\u017e 1% m\u016f\u017ee spole\u010dnosti jako celku p\u0159ipadat jako \"nic moc\", ale je p\u0159\u00edli\u0161 vysok\u00e1 na to, abychom ignorovali fakta. V EU \u017eij\u00ed p\u0159ibli\u017en\u011b 3 a\u017e 4 miliony lid\u00ed, nebo jinak \u0159e\u010deno: statisticky vzato trp\u00ed celiaki\u00ed ka\u017ed\u00fd v\u00edkend v jedn\u00e9 st\u0159edn\u011b velk\u00e9 restauraci v N\u011bmecku 2 a\u017e 4 host\u00e9. Pro tyto lidi maj\u00ed bezlepkov\u00e9 potraviny element\u00e1rn\u00ed v\u00fdznam. EU si tento probl\u00e9m uv\u011bdomuje a od roku 2009 reguluje ozna\u010dov\u00e1n\u00ed bezlepkov\u00fdch potravin.<\/p>\n\n\n\n<p>Pr\u00e1vn\u00ed r\u00e1mec pro ozna\u010dov\u00e1n\u00ed \"lepku\" v potravin\u00e1ch v EU m\u00e1 v\u0161ak pon\u011bkud pohnutou historii: prvn\u00ed na\u0159\u00edzen\u00ed (na\u0159\u00edzen\u00ed (ES) \u010d. 41\/2009) bylo na dlouhou dobu pozastaveno a teprve po (p\u0159\u00edli\u0161 dlouh\u00e9m) p\u0159echodn\u00e9m obdob\u00ed bylo nahrazeno (v sou\u010dasn\u00e9 dob\u011b platn\u00fdm)&nbsp;<a rel=\"noreferrer noopener\" href=\"https:\/\/eur-lex.europa.eu\/legal-content\/DE\/TXT\/HTML\/?uri=CELEX:32014R0828&amp;from=DE\" target=\"_blank\">Prov\u00e1d\u011bc\u00ed na\u0159\u00edzen\u00ed (EU) \u010d. 828\/2014<\/a>&nbsp;nahrazeny.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"glutensensitivit\u00e4t-ohne-z\u00f6liakie\">Citlivost na lepek bez celiakie?<\/h2>\n\n\n\n<p>V ur\u010dit\u00fdch soci\u00e1ln\u00edch skupin\u00e1ch se tak\u00e9 zd\u00e1, \u017ee popt\u00e1vka po bezlepkov\u00e9 strav\u011b je v\u00fdrazn\u011b vy\u0161\u0161\u00ed. Neofici\u00e1ln\u00ed zpr\u00e1vy hovo\u0159\u00ed o 50 % u (p\u0159ev\u00e1\u017en\u011b mlad\u00fdch) \u00fa\u010dastn\u00edk\u016f pracovn\u00edho a cestovn\u00edho programu v Kanad\u011b.<\/p>\n\n\n\n<p>To je zna\u010dn\u011b vysok\u00e9 \u010d\u00edslo, kter\u00e9 umo\u017e\u0148uje generovat tr\u017eby. Pro v\u00fdrobce \"Dr. Sch\u00e4r Group\" z It\u00e1lie je prodej \"bezlepkov\u00fdch\" v\u00fdrobk\u016f velmi lukrativn\u00edm obchodem.<\/p>\n\n\n\n<p>Na rozd\u00edl od celiakie, pro jej\u00ed\u017e existenci existuj\u00ed jasn\u00e9 d\u016fkazy a pro jej\u00ed\u017e diagnostiku existuj\u00ed jasn\u00e1 krit\u00e9ria, pro citlivost na lepek bez celiakie v sou\u010dasn\u00e9 dob\u011b (2019) neexistuj\u00ed \u017e\u00e1dn\u00e1 spolehliv\u00e1 diagnostick\u00e1 krit\u00e9ria: \u017e\u00e1dn\u00e9 biomarkery, nic skute\u010dn\u011b validovan\u00e9ho. Nesn\u00e1\u0161enlivost lepku je tedy z v\u011bdeck\u00e9ho hlediska mlhav\u00fdm pozad\u00edm.<\/p>\n\n\n\n<p>D\u016fkazy o existenci (negativn\u00ed) nesn\u00e1\u0161enlivosti lepku (citlivosti na lepek) \"pod\" hranic\u00ed celiakie jsou ve v\u011bd\u011b pova\u017eov\u00e1ny za slab\u00e9.<sup><a href=\"https:\/\/euroconsum.eu\/cs\/publication\/glutenintoleranz_getue\/#fn:3\">3<\/a><\/sup>&nbsp;S \u010d\u00e1ste\u010dn\u00fdm odkazem na d\u0159\u00edv\u011bj\u0161\u00ed studii<sup><a href=\"https:\/\/euroconsum.eu\/cs\/publication\/glutenintoleranz_getue\/#fn:4\">4<\/a><\/sup>.<\/p>\n\n\n\n<p>P\u0159esto mnoho lid\u00ed pova\u017euje \"bezlepkovou\" (nebo \"n\u00edzkolepkovou\") dietu za mo\u017enost pro sebe (a sv\u00e9 d\u011bti) na pozad\u00ed p\u0159edpokl\u00e1dan\u00e9 citlivosti na lepek. To vedlo k vytvo\u0159en\u00ed obrovsk\u00e9ho trhu, zejm\u00e9na v pr\u016fmyslov\u00fdch zem\u00edch. V segmentu trhu s bezlepkovou v\u00fd\u017eivou je italsk\u00e1 skupina Dr. Sch\u00e4r zastoupena t\u00e9m\u011b\u0159 v cel\u00e9 Evrop\u011b, Severn\u00ed Americe, Braz\u00edlii, Turecku, Indii a Izraeli. Spole\u010dnost pat\u0159\u00ed k p\u0159edn\u00edm dodavatel\u016fm bezlepkov\u00fdch potravin v Evrop\u011b a je l\u00eddrem na trhu v N\u011bmecku a It\u00e1lii.<\/p>\n\n\n\n<p>Pro lidi trp\u00edc\u00ed celiaki\u00ed znamen\u00e1 roz\u0161\u00ed\u0159en\u00ed nab\u00eddky bezlepkov\u00fdch hotov\u00fdch v\u00fdrobk\u016f nesporn\u011b obrovsk\u00e9 zv\u00fd\u0161en\u00ed kvality \u017eivota, proto\u017ee jsou zbaveni marginalizace. A co ostatn\u00ed?<\/p>\n\n\n\n<p>Ti, kdo p\u0159ech\u00e1zej\u00ed na \"bezlepkovou\" dietu bez stanoven\u00ed diagn\u00f3zy, se mo\u017en\u00e1 rozhodli \u0161patn\u011b: studie nazna\u010duje, \u017ee bezlepkov\u00e1 dieta nen\u00ed zdrav\u00e1 pro ka\u017ed\u00e9ho, proto\u017ee v\u00fdrazn\u011b zvy\u0161uje riziko vzniku cukrovky II. typu.&nbsp;<sup><a href=\"https:\/\/euroconsum.eu\/cs\/publication\/glutenintoleranz_getue\/#fn:5\">5<\/a><\/sup>.<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\"><p>\"Omezen\u00ed denn\u00ed konzumace lepku pravd\u011bpodobn\u011b nepom\u016f\u017ee v\u011bt\u0161in\u011b lid\u00ed zabr\u00e1nit vzniku cukrovky 2. typu.\" - Zong a kol.<sup><a href=\"https:\/\/euroconsum.eu\/cs\/publication\/glutenintoleranz_getue\/#fn:5\">5<\/a><\/sup><\/p><\/blockquote>\n\n\n\n<p>Studie z USA zjistila, \u017ee<sup><a href=\"https:\/\/euroconsum.eu\/cs\/publication\/glutenintoleranz_getue\/#fn:6\">6<\/a><\/sup>\u017ee 44 % spot\u0159ebitel\u016f, kte\u0159\u00ed kupuj\u00ed bezlepkov\u00e9 potraviny, <em>ne<\/em> trp\u00ed nesn\u00e1\u0161enlivost\u00ed lepku. Z nich 65% v\u011b\u0159\u00ed, \u017ee bezlepkov\u00e1 dieta je obecn\u011b zdrav\u011bj\u0161\u00ed, a 27% se obrac\u00ed k bezlepkov\u00fdm v\u00fdrobk\u016fm, aby zhublo (pro co\u017e neexistuj\u00ed \u017e\u00e1dn\u00e9 d\u016fkazy). To u lid\u00ed&nbsp;<em>bez<\/em>&nbsp;diagnostikovan\u00e1 celiakie bezlepkov\u00e1 dieta&nbsp;<em>ne<\/em>&nbsp;pozitivn\u00ed \u00fa\u010dinek, existuj\u00ed tak\u00e9 dobr\u00e9 d\u016fkazy z placebem kontrolovan\u00e9 a dvojit\u011b zaslepen\u00e9 studie.<sup><a href=\"https:\/\/euroconsum.eu\/cs\/publication\/glutenintoleranz_getue\/#fn:7\">7<\/a><\/sup>.<\/p>\n\n\n\n<p>20% \u00fa\u010dastn\u00edk\u016f s nejni\u017e\u0161\u00ed spot\u0159ebou lepku m\u011blo o 13% vy\u0161\u0161\u00ed riziko diabetu 2. typu.<sup><a href=\"https:\/\/euroconsum.eu\/cs\/publication\/glutenintoleranz_getue\/#fn:8\">8<\/a><\/sup>&nbsp;(Video z prezentace).<\/p>\n\n\n\n<p>Studie Zong et al. tak\u00e9 zjistila v\u00fdznamn\u011b zv\u00fd\u0161en\u00e9 riziko vzniku diabetu II. typu u \u00fa\u010dastn\u00edk\u016f studie bez celiakie, kte\u0159\u00ed jedli bezlepkovou dietu. S 200 000 \u00fa\u010dastn\u00edky se jednalo o mimo\u0159\u00e1dn\u011b rozs\u00e1hlou studii: 20% \u00fa\u010dastn\u00edk\u016f s nejni\u017e\u0161\u00ed spot\u0159ebou lepku m\u011blo o 13% vy\u0161\u0161\u00ed riziko vzniku diabetu 2. typu ne\u017e 20% s nejvy\u0161\u0161\u00ed spot\u0159ebou. U inverzn\u00edch z\u00e1v\u011br\u016f se doporu\u010duje opatrnost (jako v\u017edy): Studie vyvrac\u00ed podez\u0159en\u00ed, \u017ee lepek je kardiometabolick\u00fd jed. Pro lidi bez onemocn\u011bn\u00ed spojen\u00fdch s lepkem v sou\u010dasn\u00e9 dob\u011b existuj\u00ed. <em>\u017e\u00e1dn\u00e9 argumenty ve prosp\u011bch bezlepkov\u00e9 diety, ale sp\u00ed\u0161e argumenty proti n\u00ed.<\/em><sup><a href=\"https:\/\/euroconsum.eu\/cs\/publication\/glutenintoleranz_getue\/#fn:8\">8<\/a><\/sup>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"woher-kommen-all-die-positiven-erfahrungen-mit-glutenfreier-ern\u00e4hrung\">Odkud poch\u00e1zej\u00ed v\u0161echny pozitivn\u00ed zku\u0161enosti s bezlepkovou dietou?<\/h2>\n\n\n\n<ul class=\"wp-block-list\"><li>Pravd\u011bpodobn\u00fdm vysv\u011btlen\u00edm pozitivn\u00edch zpr\u00e1v o lidech, kte\u0159\u00ed zm\u011bnili stravu na bezlepkovou, ani\u017e by jim byla stanovena diagn\u00f3za celiakie, m\u016f\u017ee b\u00fdt placebo efekt nebo nocebo efekt, kter\u00fd podporuj\u00ed internetov\u00e1 f\u00f3ra a skupiny na Facebooku. Celkov\u011b je \"bez\" pova\u017eov\u00e1no mnoha lidmi za pozitivn\u00ed tvrzen\u00ed bez jak\u00fdchkoli d\u016fkaz\u016f. Tato my\u0161lenka vrchol\u00ed v esoterick\u00fdch konceptech, jako je \u00fadajn\u00e1 \"light dieta\". \"Free from\" nemus\u00ed znamenat v\u016fbec nic.<\/li><li>Kdy\u017e lid\u00e9 m\u011bn\u00ed sv\u016fj j\u00eddeln\u00ed\u010dek, jen velmi m\u00e1lo z nich se dr\u017e\u00ed v\u011bdeck\u00e9ho p\u0159\u00edstupu: nem\u011bn\u00ed pouze \"l\u00e1tku\", ale velmi \u010dasto se pou\u0161t\u00ed do rozs\u00e1hl\u00e9 my\u0161lenkov\u00e9 konstrukce stravy. Neobejdou se pak jen bez jednotliv\u00fdch aspekt\u016f, ale p\u0159evr\u00e1t\u00ed ve sv\u00e9m j\u00eddeln\u00ed\u010dku v\u0161e mo\u017en\u00e9 naruby. Je pravd\u011bpodobn\u00e9, \u017ee to bude m\u00edt sv\u00e9 \u00fa\u010dinky. Ty jsou v\u0161ak nespecifick\u00e9 a nelze je p\u0159en\u00e1\u0161et.<\/li><li>Zm\u011bna stravov\u00e1n\u00ed je \u010dasto doprov\u00e1zena zm\u011bnou dal\u0161\u00edch faktor\u016f (zm\u011bna \u017eivotn\u00edho prost\u0159ed\u00ed, nov\u00fd partner, diagn\u00f3za nemoci, kl\u00ed\u010dov\u00e9 zku\u0161enosti). Lid\u00e9, kte\u0159\u00ed zm\u011bn\u00ed sv\u016fj j\u00eddeln\u00ed\u010dek, pak p\u0159ibli\u017en\u011b ve stejn\u00e9 dob\u011b za\u010dnou jezdit na kole do pr\u00e1ce.
